Full product incident report

Did a product fail, injure someone or cause damage?

Create a structured report connecting what happened to the product, seller, marketplace listing and supporting evidence.

When to use this route

Make a full report when the incident needs context

01 · PRODUCT

Identify it

Include the product name, brand, model, serial, batch or barcode, seller, marketplace and order details.

02 · INCIDENT

Explain what happened

Describe the failure, suspected hazard, injury, near miss or damage factually, including when and how it occurred.

03 · EVIDENCE

Preserve what supports it

Add safe photographs, labels, screenshots, instructions, receipts and other records that help a reviewer understand the event.

Full incident report or Whistle?

Use the full report for a faulty or defective product involving an incident, injury, near miss, damage, technical concern or any case that needs detailed evidence and a trackable FOLO-UP reference.

Unsure whether a product fault is a safety concern? Start with the faulty-product guide →

Use FOLO-UP Whistle when you are viewing a marketplace listing and want to check for earlier listing signals or flag it quickly. A Whistle submission records a listing signal for evidence capture and controlled human review; it is not a confirmed safety finding.

After the report

Keep the reference shown after submission. It can be used to track the report, and FOLO-UP may request further evidence when that is necessary for assessment.
