Recorded hazard
The product presents a high risk of electric shock as the creepage and clearance distances are insufficient and the earthing arrangements for the product are poor. Under fault conditions, the product may become live during use, and can give the user an electric shock. Additionally, the plug supplied with the product did not meet the dimensional requirements of BS 1363 because the live and neutral pins were too close to the edge of the plug face. This could result in an electric shock whilst inserting or removing the plug from the socket. Some of the required documentation and markings were not present.
