Recorded hazard
The product has been identified as presenting a high risk of electric shock as the shutters did not fully cover the aperture. The test pin could slide past the shutters and touch live parts. Also, the plug pins are just over 2mm from the edge of the plug, less than the permissible minimum, and a fuse is not fitted. A user may touch the pins when plugging in, or unplugging the product, or otherwise access live parts through insufficient shutters, and experience an electric shock.
