Recorded hazard
All of the bells were grippable and were detachable with a lower than permitted force. Once detached, the bells are capable of fitting in a small parts cylinder and present a serious risk of choking. The plastic packaging also had average plastic sheeting thicknesses of 0.024 mm and 0.024 mm which is less than the minimum prescribed thickness and presents a risk of suffocation.
