FOLO-UP does not replace Citizens Advice, Trading Standards or OPSS. This form helps consolidate product, marketplace and hazard evidence so the case can move through triage with clearer supporting information.

What reporting means

This first form opens a FOLO-UP record. It captures enough information to identify the product, marketplace and safety concern.

We do not capture everything at this first stage because collecting a full evidence file can be stressful and time-consuming. We start with the essentials, then request more if the case is activated.

Proof of purchase or delivery
Photos of damage or failure
Injury evidence, where relevant and handled carefully
Manuals, warnings or missing safety information

How the file may be used

Once complete enough, the file may help Citizens Advice, local Trading Standards, OPSS, marketplaces, manufacturers, experts or other stakeholders understand the concern.

Reports may also support anonymised statistics that help identify patterns and inform future action on unsafe products.
